Be prepared for your visit

I’m having a medical

Please download and complete the relevant Personal Health Form (at the bottom of this page) before a medical at InterHealth and bring it with you on the day of your appointment.

I’m having vaccinations

If attending for vaccinations you will need to bring previous vaccination cards with you. If you don't have them try phoning the last clinic that you attended and ask for your record. If you don’t have any record, don't worry; our nurses will talk you through a plan. Please download and complete the Pre-immunisation Form below and bring it with you on the day of your appointment.

I need to order some medicines or travel equipment from InterHealth

We deliver medicines and travel equipment worldwide. Please visit our online TravelShop for all your essentials for global travel. Subscribers are entitled to low-cost Malarone. Ordering online is simple and easy thanks to our new online TravelShop. If you would like to arrange a collection to coincide with your appointment please call us.

One of our team is coming in for Psychological Health Appointment

If you are an HR manager or programme leader, please complete a referral form and send through to info@interhealth.org.uk

Late arrivals

Please remember that there are other patients booked in after you. Please make every effort to attend your appointment on time and allow time to complete any forms beforehand. If possible we encourage you to plan to arrive 10-15 minutes prior to your appointment time. We understand that unforeseen delays can arise and will try and accommodate genuine lateness as much as possible. However, please be advised that clients more than 15 minutes late to their appointment may be asked to reschedule.
