Vaccinations
If attending for vaccinations you will need to take previous vaccination cards with you. If you don't have them try phoning the last clinic, surgery that you attended and ask for your record. If you dont have any record, don't worry.
The IH nurse will talk through a plan. If you haven't visited InterHealth before please fill out the Personal Health Form above and bring it with you. You will also need to complete the
pre-immunisation form below:
